Antiemetic choice in Bengali

 এন্টি এমেটিকস (Antiemetics) অথবা বমির ওষুধ:🤢🤢🤮🤮

বমি অথবা বমি বমি ভাব মূলত ভমিটিং সেন্টার যে সকল ইনপুট পায় সেগুলোর বহিঃপ্রকাশ। ভোমিটিং সেন্টার বিভিন্ন জায়গা থেকে ইনপুট পায় এবং বিভিন্ন জায়গায় বিভিন্ন mediator থাকে। কোন এক বিশেষ ওষুধ কোন বিশেষ ইনপুট কে ইনহিবিট করতে পারে । তাই কোনো নির্দিষ্ট ওষুধ সকল ক্ষেত্রে ব্যবহার করা যায় না। তাহলে শুরু করি--


 vomiting সেন্টার কোন কোন জায়গা থেকে ইনপুট পায় এবং সেগুলোর mediator কি??


a. Area postrema 🧠🧠 (chemoreceptor trigger zone, CTZ)( mediated by dopamine)


 b. Vestibular apparatus 🦻👂

vestibular system–induced nausea is mediated primarily by histamine and acetylcholine 


 c. Peripheral afferents from the pharynx🦒, gastrointestinal (GI) tract, and genitals ( mediated by dopamine and serotonin)


 d. Higher cortical centers 


কিছু কমন কন্ডিশন ও এন্টি এমেটিক চয়েস:


➡️➡️Migraine headache::


Dopamine is probably a primary mediator


For headache and nausea: metoclopramide or prochlorperazine


For nausea: oral antiemetics, metoclopramide, prochlorperazine, serotonin antagonists


   ➡️Vestibular nausea or MOTION SICKNESS::🚙🚗🚒🚍

Antihistamines and anticholinergics (equally effective) 

  ❌❌Domperidone, metoclopramide hydrochloride, 5HT3-receptor antagonists, and the phenothiazines (except the antihistamine phenothiazine promethazine) are ineffective in motion sickness


➡️Gastroenteritis::🧞🧞🧞


First-line agents: dopamine antagonists ( non prokinetic if diarrhoea)


Second-line agents: serotonin antagonists


Use in children is controversial


➡️Postoperative nausea and vomiting🛌🏼🛌🏼🛌🏼🛌🏼


Prevention: serotonin antagonists, droperidol, dexamethasone


Treatment: dopamine antagonists, serotonin antagonists, dexamethasone


➡️Pregnancy🤰🤰🤰


Mild case of nausea :: ginger

Drug: antihistamine((Cyclizine,prochlorperazine, promethazine, chlorpromazine)) and phenothiazine


❓Antiemetics কখন দিব?

✅Cause যদি জানা থাকে তারপর দেওয়া উত্তম।কিছু ক্ষেত্রে Antiemetics দরকার নাও হতে পারে বা ক্ষতির কারণ হতে পারে যেমন DKA, antiepileptic overdose etc


Dopamine receptor antagonist:

Domperidone, Metoclopramide etc


Serotonin receptor antagonist:

Granisetron ,Ondansetron,Palonosetron


Antihistamine (sedation occurs)::

Cinnarizine, Promethazine etc

Anti cholinergic:: Hyoscine hydrobromide

 MBBS অনেকেই জিজ্ঞাসা করে এমবিবিএস কোন সাবজেক্ট,কোন বিষয়ে বিশেষজ্ঞ হচ্ছি,কত দিন পড়া,কি কি পড়া লাগে ইত্যাদি। সেগুলা নিয়ে কিছু কথা বলি। এমবিবিএস এ কখন ভর্তি হয়? MRCP UK, when you get the degree

Progress through the diploma 8.1 On passing Part 1 Examination Once they have received confirmation that they have passed the Part 1 examination, candidates can apply for the MRCP(UK) Part 2 Written and/or the Part 2 Clinical (PACES) Examination (in accordance with the recommendations for necessary clinical experience detailed in MRCP advice and guidance).
